mitochondriale dysfunctie

aanwezig in

alle CVS-patiënten

 

 

 

 


 

Een recente studie van Booth, Myhill en McLaren-Howard laat zich als volgt samenvatten:

 

 

Bevinding 1

 

Een recente studie van Myhill en collega's bevestigt de resultaten van een studie uit 2009:

de ATP (energie-)productie is bij alle CVS-patiënten op één of meer manieren verstoord.

 

 

Bevinding 2

 

Grofweg lijkt er binnen de CVS-patiëntenpopulatie sprake van twee groepen:

 

Groep A:

 

Patiënten waarbij de aerobe verbrandingsketen niet geblokkeerd is, maar die via de normale weg toch weinig energie produceren. De oorzaak lijkt terug te voeren tot verminderde aan-voer van ADP vanuit het cytosol naar het mitochondrium: de energiecentrale (TL OUT laag).

 

Hét compensatiemechanisme voor deze groep is versnelde glycolyse, met als gevolgen een verhoogde productie van lactaat ("verzuring") en een afname van fosfocreatine (PCr), omdat deze verbruikt wordt voor transport van ATP. Glycolyse is een zeer inefficiëntie wijze van ATP-productie (1 glucose­molecuul levert slechts 2 ATP-moleculen op, terwijl die glucosemolecuul als basis voor pyruvaat bij aerobe verbranding 32 ATP-moleculen oplevert).

 

Groep B:

 

Patiënten waarbij ATP-aanmaak via aerobe verbranding (deels) geblokkeerd is, waarschijnlijk omdat het transport van ATP van het mitochondrium, waar ADP m.b.v. zuurstof via de citroen­zuurcyclus en oxidatieve fosforylatie in ATP omgezet wordt, naar het cytosol een knelpunt vormt (TL IN laag).

 

Hét meest waarschijnlijke compensatiemechanisme voor deze groep is adenylate kinase, waarbij 2 moleculen ADP omgezet wordt in 1 ATP-molecuul (energie) en 1 AMP-molecuul, die afgebroken wordt tot IMP en ammoniak. Dit laatste heeft tot gevolg dat een deel van de ADP "uit de roulatie" verdwijnt. Aanvulling van AMP-/ADP-/ATP-voorraden kan enkele dagen duren, hetgeen langdurig "herstel" na inspanning in deze groep patiënten zou kunnen verklaren.

 

 

Bevinding 3

 

In beide groepen lijkt er sprake van problemen met het transport van ADP naar het mitochondrium en/of transport van ATP naar de cytosol. Dit transport wordt mede mogelijk gemaakt door het ATP-ADP translocase (TL)-eiwit.

 

Het functioneren van het TL-eiwit lijkt verstoord te worden door virale/bacteriële pathogenen, "cellulaire restanten", die ontstaan door oxidatieve en nitrosatieve stress, en/of chemicaliën. Vervolgonderzoek in dit kader wordt momenteel uitgevoerd door John McLaren-Howard.

 

 

Bevinding 4

 

In een studie uit 1998 bleek er ook sprake van een tweedeling, patiënten met versnelde gly-colyse/verhoogde lactaatproductie en patiënten met een ander compensatiemechanisme.

Mitochondrial dysfunction and the athophysiology of Chronic Fatigue Syndrome/Myalgic Encephalomyelitis (CFS/ME).

Abstract:

 

The objectives of this study are to test the hypothesis

that the fatigue and accompanying symptoms of

Chronic Fatigue Syndrome/Myalgic Encephalomyelitis

are in part due to defects in energy provision at the cellular level,

and to understand the pathophysiology of the defects

so that effective medical intervention can be implemented.

 

We performed an audit of 139 patients (ages 18-65)

diagnosed with CFS/ME and attending a private practice.

 

The patients and 53 normal, healthy controls

had the ATP Profile test carried out on neutrophils from a 3-ml venous blood sample.

 

This test yields 6 numerical factors

that describe the availability of ATP

and the efficiency of oxidative phosphorylation in mitochondria.

 

Other biomedical measurements, including

the concentration of cell-free DNA in plasma,

were made.

 

The results of the audit are compared with

the controls and a previous cohort of 61 patients.

 

We find that all patients tested

have measureable mitochondrial dysfunction

which correlates with the severity of the illness.

 

The patients divide into two main groups

differentiated by how cellular metabolism attempts to compensate for the dysfunction.

 

Comparisons with exercise studies suggest that

the dysfunction in neutrophils also occurs in other cells.

 

This is confirmed by the cell-free DNA measurements

which indicate levels of tissue damage up to 3.5 times the normal reference range.

 

The major immediate causes of the dysfunction

are lack of essential substrates and

partial blocking of the translocator protein sites

in mitochondria.

 

The ATP Profile is a valuable diagnostic tool for the clinical management of CFS/ME.
