Johnston:

helft van de CVS-patiënten

voldoet aan de ICC criteria voor ME

 

 

 

 


 

 

 

Uit een grootschalig onderzoek van Johnston, Staines en Marshall-Gradisnik blijkt dat

circa de helft van de mensen die aan de diagnose CVS (volgens de Fukuda-criteria)

aan de veel stringentere Internationale Consensus Criteria voor ME voldoet.

 

32,0% van de 535 patiënten met de (voorlopige) diagnose "ME/CVS" voldeed aan de Internationale

Consensus-criteria (ICC), 30,3% voldeed aan de Fukuda-criteria (niet aan de ICC), 14,6% van de men-

sen was "chronisch vermoeid", maar voldeed  niet aan de Fukuda- of aan de ICC ME/CVS-criteria, en

bij 23,2% werd een (andere) medische afwijking vastgesteld die de symptomen zou kunnen verklaren.

 

Eerder bleek al dat circa 50% van de CVS-patiënten aan de Canadese Consensus-criteria voldoet.

 

De symptomen die veel vaker bij ME/CVS ICC-patiënten voorkomen

(meer dan 40% van de patiënten, verschil met CFS/Fukuda meer dan 15%),

naast post-exertionele malaise/post-exertionale (neuro-immunologische) uitputting/malaise, zijn:

disoriëntatie (45,0%), vertraagde spraak (43,9%), problemen met lezen (45,6%), buikpijn (41,5%), om-

kering dag-nacht-ritme (49,1%), regelmatig ontwaken (41,5%), eerder ontwaken dan normaal (42,1%),

levende dromen of nachtmerries (43,9%), moeite met diepte zien (45,6%), gevoelige lymfeklieren

(43,9%), misselijkheid (42,7%), buikpijn (44,4%), orthostatische intolerantie (54,4%) en ataxie (52,6%).

 

Met betrekking tot de symptomen zijn nog drie interessante conclusies te trekken.

 

Post-exertionele (neuro-immunologische) uitputting/malaise (PENE) lijkt een veel stringenter criterium

dan post-exertionele malaise (PEM): 28,9% van de CVS/niet ICC-patiënten ervaart PEM, 15,4% PENE.

 

De diagnose SEID is, zoals al eerder aangegeven (klik hier en hier), is niet afdoende om

ME-ICC of zelfs CVS te vervangen, mede omdat "subjectieve begrippen"/vragenlijstjes niet volstaan.

 

Als we alleen kijken naar het vage begrip "niet-verfrissende slaap" voldoet slechts 36,8% van

de ME ICC-patiënten aan de diagnosecriteria van SEID en slechts 33,3% van de CVS-patiënten.

 

Volgens dit onderzoek ervaarde slechts 38,0% van de ME ICC-patiënten spierzwakte.

 

Dagen aanhoudende spierzwakte na een minimale inspanning is een essentieel (verplicht) symptoom

van ME volgens de oorspronkelijke Ramsay-criteria. Dit symptoom rechtvaardigt de M van ME.

 

Weliswaar werd niet gevraagd naar post-exertionele spierzwakte (en spierpijn), maar toch:.

deze uitkomst m.b.t. spier-gerelateerde symptomen geeft te denken...

 

 


 

 

 

Epidemiological characteristics of

chronic fatigue syndrome/Myalgic Encephalomyelitis in Australian patients.

Background:

 

No epidemiological investigations have previously been conducted in Australia

according to the current clinical definitions of

chronic fatigue syndrome/myalgic encephalomyelitis (CFS/ME).

 

The aim of this study was

to describe sociodemographic and illness characteristics of Australian patients with CFS/ME.

 

 

Methods:

 

A cross-sectional survey on the medical history of patients

enrolled in an Australian CFS/ME research database between April 2013 and April 2015.

 

Participants were classified according to Fukuda criteria and International Consensus Criteria.

 

 

Results:

 

A total of 535 patients diagnosed with CFS/ME by a primary care physician were identified.

 

The mean age of all patients was 46.4 years (standard deviation 12.0);

the majority were female (78.61%), Caucasian, and highly educated.

 

Of these, 30.28% met Fukuda criteria.

 

A further 31.96% met both Fukuda criteria and International Consensus Criteria.

 

 

There were 14.58% reporting chronic fatigue but did not meet criteria for CFS/ME and

23.18% were considered on-cases due to exclusionary conditions.

 

Within those meeting CFS/ME criteria, the most common events prior to illness

included cold or flu, gastrointestinal illness, and periods of undue stress.

 

Of the 60 symptoms surveyed,

fatigue, cognitive, and short-term memory symptoms, headaches, muscle and joint pain, unre-

freshed sleep, sensory disturbances, muscle weakness, and intolerance to extremes of temperature

were the most commonly occurring symptoms (reported by more than two-thirds of patients).

 

Significant differences in symptom occurrence

between Fukuda- and International Consensus Criteria-defined cases were also identified.

 

 

Conclusion:

 

This is the first study to summarize

sociodemographic and illness characteristics of a cohort of Australian CFS/ME patients.

 

This is vital for identifying potential risk factors and predictors associated with CFS/ME and

for guiding decisions regarding health care provision, diagnosis, and management.
