Gow:

 

Afwijkende genexpressie

bij post-viraal ME/CFS

bevestigt pathofysiologie

 

 

 

 


 

Gow, Behan ("een oude rot in het vak") en kollega's hebben de genexpressie

(de mate waarin genen aangesproken worden om specifieke eiwitten aan te maken)

van 8 patienten met postviraal ME/CFS en 7 gezonde mensen bestudeerd.

 

De konklusie van hun bevindingen kan eigenlijk niemand verbazen...

 

Gow en kollega's vonden 366 relevante genen met afwijkende expressie:

286 genen zijn "overaktief", 80 genen zijn "onderaktief".

 

Een belangrijk deel van de afwijkende genenaktiviteit duidt op

  • aktivering en ontregeling van het immuunsysteem,
    • ontsteking (waarschijnlijk aangestuurd via NF-kB): COX-1/COX-2, NO, IL-6 etc.
    • verstoorde intracellulaire afweer/Th1-response: NK-cellen, T-cel-receptoren, etc.
  • oxidatieve/nitrosatieve stress
  • (aanmaak van vrije radikalen, bijv. superoxide en peroxynitriet/ONOO)

  • apoptose
  • (zelfdoding van een cel, bijv. om verspreiding van infekties te voorkomen).

Dit sluit direkt aan bij de bevindingen van Kerr (klik hier en hier) en talloze anderen.

Opmerkelijk tenslotte:

 

Sommige pathogenen, zoals mycoplasma en borellia,

zijn in staat  om op verschillende manieren het afweersysteem "het bos in te sturen".

Het is opmerkelijk dat Gow en kollega's sterke overeenkomsten konstateren

tussen de afwijkende genenaktiviteit bij ME/CVS en die bij borellia - Lyme.

 

Gow en kollega's geven één geloofwaardige verklaring voor het feit dat

verschillende geneaktiviteit-studies vaak niet dezelfde genen vinden:

de levensduur van mRNA (messenger RNA: het eerste halffabrikaat van de eiwitproduktie)

is slechts een aantal minuten: je moet er dus snel bij zijn om het te zien!

 

In deze studie zijn een aantal genen gevonden die overeenstemmen met

hetgeen Mikovits en kollega's van het Whitemore-Peterson Institute gevonden hebben:

Jammer genoeg betreft het hier wederom een klein aantal proefpersonenen

(geen verwijt, want dit type onderzoek is kostbaar én de financiële middelen zijn beperkt).

Het zou goed zijn als deze studie op grotere schaal herhaald werd.

The classical Th1 to Th2 switch

allows intracellular pathogens to evade detection.

 

 

Immune evasion

is a well-documented mechanism

by which viruses, bacteria, parasites and cancer cells

avoid destruction by the host.

 

 

The immune modulation

in post-Lyme disease fatigue

is remarkably similar to

the gene signature presented here for

patients with CFS.

 

 

In addition,

the research data do not prove

if differentially expressed genes

are the predisposing cause or

downstream effect of CFS.

 

 

It must be borne in mind, however, that

many mRNAs have very short half-lives

(i.e. less than 2 mins) and therefore

what is observed in the microarray

is a snapshot in time and

may not necessarily

reflect the chronic biological process

inherent of this condition.

Abstract (provisional)

 

Background

 

At present,

there are

no clinically reliable disease markers for

chronic fatigue syndrome.

 

DNA chip microarray technology

provides a method for examining

the differential expression of mRNA

from a large number of genes.

 

The hypothesis was that a gene expression signature,

generated by microarray assays,

could help identify genes

which are dysregulated in patients with CFS and

so provide biomarkers for the condition.

 

 

Methods

 

Human genome-wide Affymetrix GeneChip arrays

(39,000 transcripts derived from 33,000 gene sequences)

were used to compare

the levels of gene expression

in the peripheral blood mononuclear cells of

patients with post-viral chronic fatigue (n=8) and

healthy control subjects (n=7).

 

 

Results

 

Patients and

healthy subjects

differed significantly

in the level of expression of

366 genes.

 

Analysis of

the differentially expressed genes

indicated functional implications in

immune modulation,

oxidative stress and

apoptosis.

 

Prototype biomarkers

were identified on the basis of

differential levels of gene expression and

possible biological significance

 

 

Conclusions

 

Differential expression of

key genes

identified in this study

offer an insight into

the possible mechanism of

chronic fatigue following infection.

 

The representative biomarkers

identified in this research appear

promising as

potential biomarkers

for differential diagnosis and treatment.
